Ask whether a Chronic Condition Management Plan is appropriate for you, and how many of the allied health services they will allocate to exercise physiology. Your GP prepares the plan and completes the referral.

Exercise Physiology under a Medicare Chronic Condition Management Plan
If you live with a chronic condition, your GP can refer you for exercise physiology under a Chronic Condition Management Plan. Motus delivers those sessions where you already live and move, anywhere across the Perth metropolitan area.
How a Medicare plan works with Motus
A Chronic Condition Management Plan is prepared by your GP when a condition has lasted, or is likely to last, at least six months. Alongside the plan, your GP can refer you to allied health services — exercise physiology among them — for a set number of services in the calendar year.
That allowance is shared across every allied health provider you see in the period, so if you are also seeing a podiatrist or a dietitian, the services are drawn from the same pool. It is worth asking your GP how many they are allocating to exercise physiology before you start, so the program can be planned around the number you actually have.
Because Motus is mobile, none of those sessions are spent travelling. We come to your home, your local gym or a community space, assess you in the environment you actually move in, and build the program around it.
Who a Medicare plan suits

- People living with a chronic condition that has lasted, or is expected to last, at least six months
- Anyone whose GP has raised a Chronic Condition Management Plan, or who thinks they may be eligible
- People managing conditions such as osteoarthritis, persistent pain, diabetes, osteoporosis, cardiovascular disease, respiratory conditions or other long-term health concerns
- Anyone who would benefit from supervised exercise with ongoing guidance, progression and support
- People for whom getting to a clinic every week is the part that makes a program hard to sustain
The process
Four stages, from the conversation with your GP through to the report that goes back to them.
Book an assessmentA longer first appointment covering your history, what the condition stops you doing, and a physical assessment of movement, strength and function — measured, so change is visible later.
Your history, symptoms, goals and assessment findings all guide an individualised exercise program, which is introduced and supervised at your follow-up session. We explain the purpose of each exercise, coach technique and adjust the program to suit your symptoms, capacity and goals.
Sessions progress as your capacity changes, and we report back to your GP so the program stays part of your wider care. Before the allocated services run out, we will plan with you what happens next.
